Believe it or not, the ENT undershirt was only prominently featured (on its own, without the corresponding jumpsuit worn over it) five times throughout the entirety of the show, and often briefly at that!
The first was in the pilot, "Broken Bow," as Archer was suiting up again after Dr. Phlox treated his leg.

The next was near the end of the first season in "Acquisition," during which Archer wore his jumpsuit rolled down for most of the episode. This was actually the longest look we ever got of the ENT undershirt on screen.

The ENT undershirt wasn't really seen on its own during the second season, although it was seen twice during the third season.
We saw Archer briefly putting the shirt back on after his medical consultation with Dr. Phlox (readjusting to life as a human again and all).
